# Itchy eyes: is it an allergy, and what helps?

> Itchy eyes are most often caused by an allergy to pollen, dust mites, pet hair or mould. Both eyes itch, water and turn red, often with sneezing. Do not rub them. Cold compresses, lubricating eye drops, antihistamine eye drops and staying away from the trigger bring relief in most cases.

## Why allergies make your eyes itch

When something you are allergic to lands on the surface of your eye, cells in the conjunctiva release histamine. Histamine makes the blood vessels swell and the nerve endings itch. Within minutes your eyes are red, watery and itchy, and your eyelids may puff up. The medical name is allergic conjunctivitis. It is not an infection, and you cannot pass it on.

## Typical signs of an eye allergy

- itching, often intense, in both eyes
- watery eyes
- redness
- puffy eyelids, especially in the morning
- a burning or gritty feeling
- stringy, clear mucus
- sneezing, an itchy nose or a blocked nose at the same time

## Common triggers in South Africa

- **Grass pollen.** This is the main culprit. Grass pollen is in the air for much of spring and summer, and on the Highveld the grass season is long.
- **Tree pollen,** mostly in late winter and spring.
- **Dust mites.** They live in bedding, carpets and curtains, and cause symptoms all year round that are often worst in the morning.
- **Pet dander** from cats and dogs.
- **Mould spores,** in damp rooms and after rain.
- **Irritants** such as smoke, perfume and pool chlorine, which are not true allergens but can set off the same symptoms.

If your eyes itch at the same time every year, pollen is the likely cause. If they itch all year, think of dust mites, pets or mould.

## What helps

### First, do not rub

Rubbing feels good for a few seconds and then makes everything worse, because it squeezes more histamine out of the cells. Years of hard eye rubbing are also linked to keratoconus, a condition in which the cornea thins and changes shape. When the urge comes, reach for a cold compress.

### Simple relief at home

- **Cold compresses.** A clean facecloth soaked in cold water, held over closed eyes for a few minutes, calms itching and swelling.
- **Lubricating eye drops.** They rinse pollen off the eye and dilute the allergen. Keeping them in the fridge makes them more soothing.
- **Wash your face, and your hair if you can,** after being outdoors, to remove pollen.

### Medicines

- **Antihistamine eye drops,** often combined with a mast cell stabiliser, are the most effective treatment for itchy eyes and work within minutes. Your pharmacist can advise.
- **Antihistamine tablets** help when you have nose symptoms as well. Some make dry eye worse.
- **Steroid eye drops** are for severe cases only, and only under the supervision of a doctor or a suitably qualified optometrist, because they can cause cataracts and glaucoma if they are misused.
- Avoid using "red eye" whitening drops regularly. They mask the redness and can cause rebound redness.

### Reducing your exposure

- Keep windows closed on dry, windy days and in the early morning, when pollen counts are highest.
- Wear wraparound sunglasses outdoors.
- Wash bedding in hot water every week, and consider dust-mite covers for mattresses and pillows.
- Keep pets off the bed.
- Dry laundry indoors during your pollen season.
- If you wear contact lenses, switch to glasses on bad days. Pollen sticks to lenses.

## Allergic eyes in children

Children with eczema, asthma or hay fever often have itchy eyes too. Watch for constant rubbing, blinking and dark circles under the eyes. Persistent, severe itching with thick, ropey mucus in a child needs to be seen by a doctor, because a more serious form of allergic eye disease can affect the cornea.

## When to see an optometrist or doctor

- the itching is not controlled by pharmacy treatment
- only one eye is affected
- you have pain, sensitivity to light or blurred vision
- there is thick yellow or green discharge
- symptoms last for weeks or keep returning
